Are you a carer? Social Prescribing support available

Are you a Carer, or help look after someone? Could our PCN Social Prescribing service help you? If you are 18+, you can access free support from one of our Social Prescribing Link Workers.


Social Prescribing is a non-medical service that aims to improve health and wellbeing by connecting people to the information, services, groups and activities which focus on the things that matter to them.


Carers can access help on assessments, benefits advice and respite support, connecting to emotional and peer support, and connecting to vital health screening opportunities and services such as NHS Health Checks, cancer screening, and flu, Covid-19 and pneumococcal vaccinations for carers and those they look after.
